Today I found out I was not getting Alert pop-ups in the bell icon on the site, I had to check the watched threads page to see updates.

I'm just gonna copy my answer to someone else with the same problem here:

If your alerts aren't working properly you need to adjust your preferences.
I used to have some trouble myself, but my current settings do fine.

My Preferences are:
Alerts page options: Ticked
Alert pop-up options: Use per-alert configuration
Allerts summarize threshold: 4, all three options ticked
Alerting preferences: Use custom
Adds a collaborator to a thread - untick (both)
Removes a collaborator from a thread - untick (both)
The thread starter replies to a watched thread - Alert: Yes, Pop-up auto-read: No
Posts in a watched forum - untick (both)
Replies to a watched thread - untick (both)
For the rest down there I have both boxes ticked.

(Oddly enough I don't usually get any notifications for when the author / thread starter posts something in a thread.
Just when they add a threadmark.
I don't know if that's borked or what, but the alerts didn't work properly (read: as I wanted them to) without that box ticked.)
