So as the title says, this is something I've had happen to me multiple times at this point, which seems to point to it being an actual bug rather than a one off thing where I just forgot to threadmark it or something.

Essentially, when I'm going about making a new story thread and posting up multiple chapters at once, the threadmark system seems to not register the threadmarks I make in the post UI after the first 2-3 threadmarks.

I basically go about it like this:

1) Post Thread.
2) Threadmark Chapter 1
3) Copy/Paste Chapter 2, Threadmark it as Chapter 2 in the pre-post interface, and then post it all at once.
4) Rinse and Repeat for any other chapters after that, however, at some point, usually on chapter 3 or 4, Threadmarking it in the pre-post interface no longer seems to register with the system.

Eh *shrugs* happens. It's why we tell you when we come across it.
