Shiro did. Shiro isn't really a gamer at all, he's more like a reality warper with severe delusions of grandeur, so his reality warping took on the form of a gamer system because he's a fucking nerd. The game even mentioned that it was him early on. Kuro and Isis are two different entities, the Game and Shiro are the same person arguing with himself.
